At Smith Family Chiropractic, we accept all forms of insurance and as a service to you; we will submit claims to your insurance company at no charge. We also handle workman’s compensation and personal injury claims.
A list of insurance companies we are in network with is listed in next section. Check to see if we are in or out of network with your insurance company.
If you do not have insurance we offer a discount which is subject to our self pay policy, explained below.
In/out of Network:
Insurance Companies we are in network with: Blue Cross Blue Shield, Medicaid (MA), Medica, Great West Healthcare, PacificCare, United Healthcare and Lakes Area Healthplan.
Medicare: we are non-participating with Medicare. We will still submit claims for you and you will still have chiropractic benefits, however, Medicare will pay you any benefits that are available. As a result, you will be responsible to pay the Medicare allowed amount for your service at each visit.
If we are out of network with your insurance company, you can still receive care in our office and we will submit claims to your insurance company, however, your visit will be subject to your out of network benefits.
Self Pay Policy:
For those without insurance or if you choose not to have claims submitted to your insurance company, your visit will be subject to our self pay policy. HSA or flex dollars can be utilized for payment for these services as well.
We offer a generous discount off your procedures when your services are paid on the same date your services are rendered. If you choose not to pay on the same date of service, your services are subject to our full price fee schedule.
All services associated with our FIrstLine Therapy program are not sent to insurance. Payment for these services is due at the time services are rendered.
Many times treatment for neurological conditions is not covered under specific health plans. In these cases, payment will be subject to our self pay policy.
At Smith Family Chiropractic, we are accepting new patients of all ages. It is important when you call to make an appointment to let our staff know what it is you will be coming in for. We offer a wide variety of services and we schedule appointments a bit differently depending on the services needed. Call To Set An Appointment Phone: 218-736-4113
Check out the different appointment types below:
Dr. Kelly Smith does the vast majority of our acupuncture. This modality is effective for a long list of health issues, but is most commonly used for pain control in our office.
It is also important to note, that our doctors’ acupuncture license is tied to their chiropractic license, so any acupuncture services need to be done in conjunction with a chiropractic adjustment.
Our doctors have practiced traditional chiropractic care since 1997 and offer great care with a very thorough exam and holistic approach. If you are a new patient in our office, you first visit will be a longer appointment time in order to establish you in our office. A very thorough history and examination will be completed to assure that you get the most complete care.
If you are a current patient with no new injuries, we do our best to accommodate your schedule.
If you have not been seen in our office for more than two years, we require a new examination in order to re establish you as a current patient in our office.
If you are seeking care in our office for a neurological disorder, concussion, brain injury, or learning disability your appointments will be a bit longer. Your initial appointment will likely consist of some testing with our CAPS machine and complete VNG exam. The doctor will also review your history in detail and complete a thorough diagnostic exam of your neurological health.
Next, the doctor will complete a treatment plan based on the results of the examination and testing. Some of the treatment plan will involve in office rehabilitation techniques, however the majority of the work will be up to the patient to complete after the office visits. Exercises will be given to perform at home, often several times a day. Simply coming to the appointments and receiving in office treatment will not be enough to create permanent change.
Follow up appointments will be scheduled as needed according to each patient’s specific needs. At times a periodic check in every couple of weeks or months will be sufficient. Other times, our doctors may opt for a more intensive approach where the patient will be seen several times per day for a few days.
If you are seeking an appointment in order to create a healthier lifestyle we would schedule two appointments for you. The first appointment will be for your BIA or bioimpedance analysis. We would set up your second appointment a couple of days later to meet with Dr. Ala. At this time, she will go over your BIA results and discuss a personal plan for your wellness.
This program offers great results, but requires work on the patients’ part. Healthy living is certainly not easy and there is no magic bullet, but feeling great and avoiding disease every day is worth every bit of effort.
